ABSTRACT

Abstract Objective: To evaluate the impact of daily toothbrushing frequency on parental reports of dental pain and discomfort in preschoolers. Material and Methods: An observational cross-sectional study was conducted involving 376 children between four and five years of age at public preschools in the city of Campo Magro, Brazil. Parents/guardians answered a socioeconomic questionnaire that contained a single question on the child's daily toothbrushing frequency and the Brazilian version of the Dental Discomfort Questionnaire (DDQ-B). Statistical analysis involved bivariate and multivariate Poisson regression analyses (α=0.05). Results: After the adjustments in the multivariate analysis, the prevalence of dental pain and discomfort was lower among children whose last visit to the dentist was for prevention (PR = 4.42; 95% CI: 1.75- 11.14; p=0.002) and those with a higher daily toothbrushing frequency (PR = 2.13; 95% CI: 1.12-4.05; p=0.021). Conclusion: A lower toothbrushing frequency is associated with parental reports of dental pain and discomfort in preschoolers. Educational and preventive measures that stimulate an increase in daily toothbrushing frequency can contribute to a better oral health status in preschoolers, consequently, less prevalence of dental pain and discomfort.

ABSTRACT

Objetivo: o objetivo deste trabalho é relatar um caso clínico de hipersensibilidade severa associada a múltiplas lesões cervicais não cariosas, entender o mecanismo de ação e a eficácia dos agentes dessensibilizantes. Relato de caso: paciente do gênero masculino, 38 anos de idade, compareceu à clínica da Faculdade Morgana Potrich com um quadro de hipersensibilidade em vários elementos, bem como, a presença de lesão cervical não cariosa em alguns desses elementos. Durante a anamnese o paciente relatou o hábito de escovar os dentes várias vezes ao dia logo após se alimentar, exercendo muita força durante o ato, relatou também fazer consumo diário de cerveja que possui um pH ácido. Ao realizar análise da oclusão notou-se um desequilíbrio oclusal e contato prematuro nos dentes 24 e 34. O tratamento de escolha para esse caso foi o ajuste oclusal seguido do uso de dessensibilizantes e restaurações em resina composta nos dentes que havia a presença de lesões cervicais não cariosas com perda de estrutura dentária. Considerações finais: após esse tratamento, realizado em várias sessões, foi possível devolver ao paciente a função e eliminar a dor provocada pela hipersensibilidade, proporcionando uma melhor qualidade de vida para esse paciente.


Objective: This study aims to report a clinical case of severe hypersensitivity associated with multiple non-carious cervical lesions and to understand the mechanism of action and the efficacy of desensitizing agents. Case report: A 38-year-old male patient attended the clinic at Faculdade Morgana Potrich with the hypersensitivity of several elements and the presence of non-carious cervical lesion in some of these elements. During anamnesis, the patient reported the habit of toothbrushing several times a day soon after eating, exerting a lot of force in such activity; he also reported the daily consumption of beer, which has an acidic pH. The occlusal analysis showed occlusal disorder and premature contact in teeth 24 and 34. The treatment of choice for this case was the occlusal adjustment followed by the use of desensitizers and composite resin restorations on the teeth with non-carious cervical lesions with loss of dental structure. Final considerations: After this treatment, which was performed in several sessions, it was possible to return function to the patient and eliminate the pain caused by hypersensitivity, providing a better quality of life for this patient.(au)

Introdução: Autopercepção em saúde abrange a compreensão que o indivíduo possui sobre o seu próprio estado de saúde, baseando-se em experiências anteriores e no contexto social, cultural e histórico em que está inserido. Objetivo: Analisar a autopercepção em saúde bucal de adolescentes e adultos jovens com 17-21 anos no município de Sobral-CE, relacionando-a com a utilização de serviços odontológicos e prevalência de dor de dente. Métodos: Estudo transversal aninhado à terceira onda da coorte de saúde bucal, desenvolvida no ano de 2012, com 482 participantes, a partir de questionário validado pelo Projeto SB Brasil 2010. Resultados: Houve associação entre classificação da saúde bucal autorreferida com mastigação, aparência de dentes e gengiva e necessidade de tratamento. Além disso, autopercepção sobre a mastigação teve associação com dor de dente e a necessidade de tratamento autorreferida com a utilização de serviços e dor de dente. Conclusões: A percepção sobre saúde bucal de adolescentes e adultos jovens está relacionada com a estética, função mastigatória e a necessidade de tratamento. Esse quadro sinaliza a preferência da utilização dos serviços odontológicos em casos de dor em detrimento de procedimentos preventivos. Seria importante que o planejamento em saúde bucal fosse orientado não somente por dados normativos, mas que incluísse a percepção da população para que as ações propostas fossem mais coerentes e aceitáveis pelos usuários (AU).


Introduction: Self-perceived health is related to the health knowledge that individuals have about their own status based on both previous experiences and the social, cultural and historical context in which they are inserted. Objective: To analyze the self-perception of oral health of adolescents and young adults aged between 17-21 years in Sobral-CE and its relation to the use of dental services and prevalence of toothache. Methods: Cross-sectional study nested to the third wave of an oral health cohort, developed in 2012, with 482 participants aged between 17-21 years, with a questionnaire validated by a National Oral Health Survey in Brazil, the Project SB Brasil 2010. Results: There was association between self-reported rating of oral health with chewing, appearance of teeth and gums and treatment need. In addition, mastication perception was directly associated with toothache and self-reported need treatment was associated with the use of services and toothache. Conclusions: The perception about oral health in adolescents and young adults is related to aesthetics, masticatory function and treatment need. This situation indicates the preference by using dental services in cases of toothache instead of preventive procedures. It is important that oral health planning services should not only be guided by normative data, but includes the perception of the population so that the proposed actions would be more consistent and acceptable from patients (AU).

Analisou-se a velocidade de movimentação ortodôntica e sensibilidade dolorosa de caninos superiores e inferiores submetidos à irradiação com o laser de baixa intensidade (LBI) em dose única mensal, comparados aos caninos contralaterais não irradiados (controle). Foram selecionados 11 indivíduos, portadores de má oclusão de Classe I com indicação de extração dos primeiros pré-molares superiores e inferiores. O laser de baixa intensidade foi aplicado após a ativação da retração inicial dos caninos, realizada através de mola Niti com uma força de 150g. A aplicação do laser foi realizada em 10 pontos (5 por vestibular e 5 por lingual) nos caninos superiores e inferiores, utilizando o seguinte protocolo: Canino inferior por vestibular e lingual e canino superior por vestibular: Potência de 40mW; densidade de energia de 10J/cm2; 10 segundos por ponto; energia por ponto de 0,4J, uma energia total de aplicação de 4J para o canino inferior e 2J por vestibular dos caninos superiores e para o canino superior por palatino o protocolo foi: Potência de 70mW; densidade de energia de 35J/cm2; 20 segundos por ponto; 1,4J de energia por ponto, energia total de 7J. A retração durou 3 meses, num total de 3 aplicações de laser. Eleven individuals with Class I malocclusion with indication of extraction of the first maxillary and mandibular premolars were selected. The low-level laser was applied after the activation of initial retraction of the canine teeth, performed with Niti coil springs with force of 150g. The laser application was performed in 10 points (5 buccal and 5 lingual) in the maxillary and mandibular canines, using the following protocol: Mandibular canine buccal and lingual and maxillary canine buccal: 40mW power; an energy density of 10 J/cm2; 10 seconds per point; energy per point 0,4J, a total energy of 4J applying to the mandibular canine and 2J the buccal of the maxillary canines and maxillary canine palatally: 70mW of power; an energy density of 35J/cm2; 20 seconds per point; 1,4J energy per point, the total energy 7J. ABSTRACT

This prospective randomized, double-blind factorial study aimed to compare the efficacy of three different intracanal medicaments with the placebo in controlling the postoperative pain after complete root canal preparation. The study was performed on 64 mandibular molars of 64 patients with diagnosis of pulp necrosis and acute apical periodontitis. After chemomechanical procedures using the stepback technique and 1% sodium hypochlorite, the teeth were randomized into four treatment groups (n=16). In group I, canals were filled with calcium hydroxide paste mixed with 2% chlorhexidine gel, group II received 2% chlorhexidine gel, group III was treated with calcium hydroxide paste, and group IV received no dressing (control). Before dismissal, preoperative pain experience was recorded using a visual analog pain scale. Patients were then instructed to quantify the degree of pain experienced 4 h after treatment and daily for a further 24, 48, 72 and 96 h. Two-way repeated measures ANOVA test and post hoc Tukey's HSD test revealed that at each time interval groups I and II were significantly more effective in reducing the postoperative pain values than groups III and IV (p<0.05). Dunnett's test showed that groups I and II differed significantly from control whereas difference between group III and control was not significant (p>0.05). Patients with pulp necrosis and acute apical periodontitis that had been dressed with chlorhexidine alone and calcium hydroxide plus chlorhexidine gave rise to less pain than that experienced by patients who had a calcium hydroxide dressing alone or no dressing at all.


Este estudo prospectivo randomizado, duplo-cego, fatorial teve como objetivo comparar a eficácia de três diferentes medicamentos intracanal com o placebo no controle da dor pós-operatória após a preparação completa do canal radicular. O estudo foi realizado em 64 molares inferiores de 64 pacientes com diagnóstico de necrose pulpar e periodontite apical aguda. Após os procedimentos químico-mecânicos com a técnica escalonada (stepback) e hipoclorito de sódio a 1%, os dentes foram divididos aleatoriamente em quatro grupos de tratamento (n=16 por grupo). No grupo I, os canais foram preenchidos com pasta de hidróxido de cálcio misturado com 2% de clorexidina gel, grupo II receberam 2% de clorexidina gel, grupo III foi tratado com uma pasta de hidróxido de cálcio e do grupo IV não receberam curativo (controle). Antes de liberar o paciente, a sensação de dor pré-operatória foi registrada com uma escala visual analógica. Os pacientes foram instruídos para quantificar o grau de dor experimentada após 4 h de tratamento e diariamente após 24, 48, 72 e 96 h. Os testes ANOVA a dois critérios para medidas repetidas e teste de Tukey post hoc HSD revelaram que, a cada intervalo de tempo, o grupo I e grupo II foram significativamente mais (p<0,05) eficazes na redução da dor pós-operatória que os grupos III e IV. Além disso, o teste de Dunnett mostrou que o grupo I e grupo II diferiram significativamente com o controle enquanto que a diferença entre o grupo III e controle foi não significativo. Pacientes com necrose pulpar e periodontite apical aguda que receberam curativos de demora de clorexidina e hidróxido de cálcio mais clorexidina apresentaram menos dor do que aqueles que receberam curativo de hidróxido de cálcio ou não receberam qualquer curativo.

Este estudo investigou os efeitos do laser em baixa intensidade na movimentação ortodôntica com a intenção de promover a ancoragem e a inibição da dor pós-ativação, bem como a verificação da preservação da integridade tecidual. A amostra constou de um total de 74 molares e pré-molares superiores e inferiores que serviram de ancoragem na fase de retração inicial de caninos realizada com molas de NiTi, com força de 150g. Um dos molares/pré-molares escolhido aleatoriamente foi irradiado com laser de diodo (LI), seguindo o seguinte protocolo de aplicação: 780mm/70mW/105J/cm2/4,2J por ponto/Et= 42J para os molares e 42J para os prémolares nos dias 0, 7 e 14 pós-ativação, sendo que o contralateral foi considerado placebo (LNI). A retração durou em média 3 meses, num total de 9 aplicações de laser. Utilizou-se modelos de gesso e tomografias computadorizadas do feixe cônico (TCFC) para a verificação da perda de ancoragem dos molares. Os modelos adquiridos a cada mês foram medidos com paquímetro digital e as tomografias adquiridas nos tempos iniciais (T0) e finais (T4) foram analisadas por meio da análise 3D (Radiomemory). Nas tomografias foi também avaliada uma possível reabsorção na crista óssea alveolar (CA), por meio da distância desta (CA) até a junção cementoesmalte (JCE), comparando-se T0 e T4 e a quantidade de reabsorção radicular, por meio da diferença de comprimento radicular inicial e final. Para avaliação da sintomatologia dolorosa comparando-se o LI e o LNI foi utilizado uma escala visual analógica após 12, 24, 48 e 72 horas após a irradiação, na qual os pacientes marcavam de 0 a 10 em consonância com a dor experimentada. This study investigated the effects of low level laser in orthodontic movement, aiming to promote anchorage and inhibition of pain after activation, as well as to analyze the maintenance of tissue integrity after utilization of laser and orthodontic movement. The sample was composed of 74 maxillary and mandibular molars and premolars that served as anchorage in the initial retraction stage of canines, performed with NiTi coil springs with force of 150g. One of the randomly selected molars/premolars was irradiated with diode laser (LI), according to the following application protocol: 780mm/70mW/105J/cm2/4.2J per point/Et= 42J for molars and 42J for premolars at days 0, 7 and 14 after activation, and the contralateral tooth was considered as (LNI). The retraction lasted 3 months in the average, adding up to 9 laser applications. Dental casts and cone beam computed tomographies (CBCT) were evaluated to analyze the anchorage loss of molars. The dental casts obtained every month were measured with a digital pachymeter and the tomographies obtained at initial (T0) and final periods (T4) were assessed by 3D analysis (Radiomemory). The tomographies were also assessed to evaluate a possible resorption of the alveolar bone crest (CA), by measuring the distance between it (CA) and the cementoenamel junction (JCE), comparing T0 and T4 and the quantity of root resorption, by the difference between the initial and final root lengths. The painful symptomatology was analyzed by comparison of LI and LNI using a visual analogue scale after 12, 24, 48 and 72 hours, in which the patients indicated from 0 to 10 according to the pain experienced. Los pacientes con pulpitis aguda, producto al dolor que las caracteriza, en ocasiones no permiten realizar un tratamiento endodóntico de urgencia satisfactorio. Objetivo: evaluar la acción analgésica de la digitopuntura previo al tratamiento estomatológico convencional en las pulpitis agudas. Método: se realizó un estudio de evaluación, cuasi-experimental de tipo antes y después, en la consulta de urgencias estomatológica del policlínico Previsora desde septiembre de 2008 hasta abril de 2009, el universo de estudio se conformó por los pacientes que acudieron a la consulta con dolor producido por pulpitis aguda, la muestra quedó se integró por 40 pacientes que no habían recibido tratamiento previo. Se evaluó la intensidad del dolor a los 30min y se valoró la cooperación del paciente durante el tratamiento convencional. Resultados: en el 90 por ciento de los casos se evaluó como eficaz el tratamiento realizado y los pacientes refirieron sentirse satisfechos.


Patients with acute pulpitis caused to the pain that characterizes it, in occasions allow carrying out a satisfactory endodontic treatment of urgency. Objective: to evaluate the analgesic action of digitopuncture previous to the conventional stomatological treatment in acute pulpitis. Method: a before and after type quasi-experimental, evaluation study was performed, in the stomatological urgency consultation at Previsora polyclinic from September 2008 to April 2009, the study universe was conformed by patients that came to the consultation with pain caused by acute pulpitis and the sample was integrated by 40 patients who had not received previous treatment. The intensity of pain was evaluated to 30 minutes and patient's cooperation was valued during the conventional treatment. Results: in 90 percent of cases was evaluated as effective the treatment carried out and patients referred to feel satisfied with the treatment received (90 percent).

ABSTRACT

El presente artículo aborda el concepto de dolor discriminando su dimensión orgánica de su dimensión psíquica, para estudiar, a partir de allí, el lugar que este fenómeno tiene en la instauración del lazo social entre odontólogo y paciente. Se definen los conceptos de demanda de alivio y demanda de curación, analizando las consecuencias de cada una en la práctica clínica en odontología. Por último, se indica el rol del odontólogo en la operación de transformación de la demanda de alivio en demanda de curación, con el fin de optimizar las estrategias clínicas en la práctica odontológica

ABSTRACT

The main objectives of this study were to determine the incidence and degree of interappointment pain associated with the use of various irrigants and medicaments utilized during endodontic treatment and to determine, through statistical analysis, if a significant relationship existsed between such pain and various conditions of the treated teeth. This prospective clinical study was composed of 318 patients who presented for endodontic treatment in the department of Dentistry at Prince Rashid Ben Al Hassan Hospital between 1991-1994. Patients age ranged from 14-67years; 177were males and 141 were females. The patients were randomly allocated to three groups to which the chemical agents to be used for irrigation and intracanal medication were assigned. Formolcresol was used in the treatment of patients in group 1, non-setting calcium hydroxide paste in group 2, while patients in group 3 received no intracanal medicament [control group]. Sodium hypochlorite solution [2.6%] was used as an irrigant in both group 1 and group 2 patients, while physiologic normal saline solution was used in group 3 patients. Instrumentation of the root canals was completed at the first visit. All endodontically treated root canals were filled by lateral condensation of gutta-percha with Seal Apex root canal sealer. The incidence and degree of pain experienced by the patients during the course of root canal therapy were assessed. Of the 318 patients evaluated, 244 [76.7%] had no interappointment pain, 52 [16.4%] had slight pain, and 22 [6.9%] had moderate to severe pain. There was no significant difference in the incidence and degree of interappointment pain between patient groups or between any of the groups in respect to teeth which had initially been vital, non - vital, intactor opened previously for pain and teeth with and without periapical radiolucent areas. In vital teeth, the incidence of interappointment pain was 35.5% in group 1patients, 20% in group 2 and 23.3% in group 3 patients. Regarding non-vital teeth, the incidence of interappointment pain was 29.5% in group 1 whereas in group 2 and 3 it was 14.7% and 20.9% respectively. The clinical use of either formolcresol or non-setting calcium hydroxide paste as an intracanal medicament in conjunction with the irrigant sodium hypochlorite [2.6%] solution for treatment of maxillary central incisors does not cause an increased incidence or degree of interappointment pain compared with normal saline solution irrigant without an intracanal medicament. In patients beginning endodontic treatment in those teeth with no symptoms, the occurrence of interappointment pain is not clinically predictable
